Abatement Notification Guide
 

 

If one plans to perform any lead abatement activities, it is required that a written/online notice be submitted to IDOH via postmark, hand deliver or electronically at least two (2) working days before the start date of such activity.  
 

 

A current form found on our website, must be filled out. The information on the document is required.  
 

 

Please complete abatement notification details and attach Occupant Protection Plan, along with the $50 fee authorize by providing credit/debit card details, allowing at least two (2) working days before the start of a lead-based paint activity.  
 

 

Once received, an approval will be issued – no work shall be given without this approval.
 

 

Listed dates shall be as close to actual work dates as possible. Work can only occur on listed work dates (not before nor after listed work dates).
 

 

Intentional avoidance of a state inspection may result in enforcement actions.
 

 

A project modification can occur when there is a change in the scope of work or the scheduled work dates or times. In this instance, a revised notification is required to be submitted to the Division (Work shall only ensue upon receipt of the new approval). No abatement activity is permitted outside of the dates on the abatement notification and approval letter from the division.
